Evening Clinical Pharmacist – Hospice Care

Overview

We are seeking a pharmacist to support structured evening operations at Compassionate Care Pharmacy. This role combines clinical consultation with evening pharmacy workflow, helping ensure hospice nurses and care teams have timely access to medications and pharmacist guidance when urgent needs arise. Pharmacists in this role assist with medication verification, coordinate evening delivery logistics, support new patient admissions, and help prepare the pharmacy for next-day workflow. This position offers a structured evening schedule, meaningful clinical interaction with hospice care teams, and the opportunity to play a key role in supporting patient comfort and dignity.

Compensation

Full-Time Pharmacists
Starting salary from $115,000 annually, with benefits.

Part-Time Pharmacists
Starting at $60 per hour for evening shifts.

Overnight On-Call Compensation

Weekdays — $100 per shift
Weekends — $150 per shift

Schedule

Evening Shift (On-Site)
4:00 PM – 10:00 PM

Weekday shifts include a 4 PM – 6 PM overlap with the daytime pharmacist to assist with verification and workflow transition.

Weekend shifts begin at 6 PM and do not include the overlap period.

Overnight On-Call Coverage
10:00 PM – 6:00 AM

 

Overnight coverage may include nurse consultations, coordination with delivery drivers, and occasional medication fulfillment if urgent preparation is required.

Most overnight activity occurs between 10 PM and 11 PM, and requests after midnight are rare (approximately once per month).

Responsibilities

  • Provide clinical guidance to hospice nurses regarding comfort care medications and therapeutic options

  • Triage RN calls and coordinate after-hours medication requests

  • Assist daytime pharmacist verification during the 4 PM – 6 PM overlap period

  • Ensure evening medication orders are completed and routed for delivery

  • Coordinate logistics with delivery drivers and hospice teams

  • Process and prepare next-day medication orders

  • Complete new patient admission intake and setup in pharmacy software

  • Prepare unit-dose syringes for facility orders when needed

  • Monitor and manage pharmacy inventory including medications, syringes, and packaging supplies

  • Prepare next-day workflow updates for the daytime pharmacy team

  • Maintain accurate documentation and pharmacy safety standards

Qualifications

  • Doctor of Pharmacy (Pharm.D.) from an accredited institution

  • Active Oklahoma pharmacist license

  • Experience in a pharmacy practice setting

  • Strong communication and clinical decision-making skills

  • Ability to collaborate with nurses, prescribers, and care teams

  • Familiarity with hospice or palliative care medication management encouraged but not required

  • Experience with PioneerRx pharmacy software preferred but not required
